No. The cover inside the book doesn't change when you change the cover in the calibre library or send a cover to the device using kobo utilities. If you change the cover in calibre (edit metadata etc) and want the cover to change in the book then you must use polish, embed metadata, modify epub plugin, the calibre editor, or perhaps other methods.

For example, the following sequence does not change the cover in the book.
  • Add the book to calibre.
  • Send the book to the device.
  • Update the cover in calibre.
  • Send the new cover using kobo utilities.

The cover inside the book is normally updated to the cover in calibre's library when you send the book to the device. If you want the cover inside the book on the device to change then you must resend the book after updating the cover in the calibre library.
