It will compress all images, including the cover.

If you want to do a lossy compression on all or select images, you can use the Editor in Calibre and select "Tools > Compress Images Losslessly", and then the subsequent dialog box has the option to "Enable lossy compression of JPEG images" and allows you to select a target percentage under "Image quality".
