Yes, this does happen sometimes, it's harmless and has always been more trouble to track down than its worth, at least to me.
The cover getting re-rendered is supposed to happen, that is because the metadata in the book details panel can get updated when the device is connected so it is essentially refreshed. However, sometimes the cover ends up as the cover from the on device copy of the book, it's basically a timing issue in the race between the book details being refreshed and the device scan being completed.
