Thanks for the info.
It seems that Sigil ignores the first entry.
I changed the DOCX so "Chapter One" is the first entry and I got this:
<h2>chapter one</h2>
<h2 id="sigil_toc_id_1">chapter two</h2>
This is an epub3.
I'm going to try creating a ghost first entry and see how it looks.
