The current version of this plugin does not support replacing the cover of PDF-backed print replica books. If you restart calibre in debug mode you will see the error message produced when this is attempted.

The next plugin release will allow changing of the cover image used for the library view thumbnail and device lock screen. However it will not change the first page in the flow of the book to match the new cover. That is the best I can do for now.
