If science fantasy is acceptable, then The Cyborg and the Sorcerers by Lawrence Watt-Evans would be my recommendation.

If you want something closer to Modern Fantasy, then On a Pale Horse, the first of the Incarnations of Immortality series by Piers Anthony.

In terms of epic or heroic fantasy, I'm not sure I have one that is a particular favorite.
