Quality check set the Calibre Metadata. It does not change the Book. For that you need to Polish.
When you Add, Calibre normally reads the book (OPF) unless you have set Add to use filename.

CopyToLibrary will add a new entry if there is no match, OTOH it will add the book to the existing (wishlist) entry, but frequently fails to update the empty fields

I suspect there is a conflict in the books (per file) metadata with the OPF.
