I have a large epub document (approx 400 pages of A4) which... I feel I ought to generate an index for. While I could do it manually, that would be a lot of work. Just manually entering the <a> tags for each page and sorting out the corresponding pagemap file is more work than I really fancy for a vanity project.

Does anyone have any guidelines or pointers for tools that can do this? Searching on these forums for 'index' and 'indexing' and a quick look in the 'Related Tools' and 'Ideas' threads hasn't put me on to anything. I'm really hoping I've just overlooked an ideal fix.

The book is primarily epub2, but has been manually massaged to convert nicely to pdf. Doesn't exist as a print form. I don't especially mind converting to epub3 if necessary, but, eh, it all works as 2 so I stuck with the lower version in the hope of slightly better compatibility.

Just an automatic or semi-automatic 'pagination tool' would be nice, and a 'generate index' tool would be a lovely bonus, but I suspect I might be asking too much.
