If you're using the calibre library, the metadata in it has precedence over the metadata you're editing with the ebook-editor. In that case, change the setting in the editor : preferences / integration with calibre / update metadata embedded (there are quite a few threads on this subject, search in this forum).

If your book is not in the calibre library (i.e. you're editing it with the stand-alone "ebook-edit"), then maybe your cover file has some error. Try to check the epub (menu tools / check book)
