It would be a username (either just username or domain/user) and password that has rights on the server you are trying to connect to. The IP would be the IP address of the server.

However, the program you are asking about is pretty much an antique now (the last message before your post was from May, 2014. Most Windows server would now be using CredSSP and I'm rather doubtful that this client supports that.

If you don't have credentials, you could always ask an IT person. He likely would love a good laugh.
