My suspicion on what caused the issue @lolxnn was the fact that I think there was still an update temp file in the root of my kindle while doing the OTA update. Because the update only went through after I left the kindle idling for a moment.
So I think if the kindle is:
1. connected to USB
2. has a partial update file in the root
3. left idle (so no interaction with screen)

it will lead to the kindle auto completing the update that was partially done despite being in airplane mode and having the OTA installed.
So once the OTA completes you must immediately unplug AND also make sure that every time you plug in the kindle, delete any suspicious update files.
