This is typical of database corruption on the kobo.
The easiest way to cause database corruption is to add a bunch of books to the device over USB and then unplug the USB without ejecting. But that's not the only way.
There are bugs in the kobo firmware that occasionally cause the kobo filesystem to be shared between the USB and things running on the kobo itself. When this happens, database and possibly filesystem corruption are possible.
Different versions of the firmware, especially older ones, are worse at this. The problem persists in newer firmware, but it is getting harder and harder to reproduce this issue as the bugs get fixed.
Some people say that exiting the book and going to the home screen fixes this (or maybe makes it less likely). The only proven way so far to totally prevent the issue is to reboot the device before connecting to USB. I use the NickelMenu kobo patch extension to add a menu entry that turns off wifi and reboots my device, which makes this really easy.
