How slow? Press 'OK' and count: "One and Two and Three.............." generally get up to Eight or Nine.
And it's changes to any column… e.g. comments, not just tags. Attribution to disk i/o is OP's speculation.

Not sure if the OP has tried Windows Safe mode. As you would know, interference from third party software is a common cause of these sort of issues… AVs, access preventers protectors, search indexers, cloud syncers etc.
