That KUAL post says "Extensions should always be packaged this way (i.e., simply unpack them at the root of your device), but if they're not, unpack them inside the extensions directory instead."
That's not the case for KOL: instead, unpack the .tar.xvf file on your computer and copy the relevant install.bin file to the kindle's mrpackages directory. (Note: I'm not sure what the KOL hotfix file is for...) Then eject and run KUAL -> Helper -> Install MR Packages.
For the coverset extension, unzip it in the kindle's extensions directory (or unzip it on your computer and copy the resulting directory).
