Other metadata in that KFX file shows that it was produced using KFX Output. KFX files produced that way can have "approximate page numbers" added during the conversion process. In the next release of KFX Input I will add an indication in the conversion log to make it clear when the removal of approximate page numbers has been done.

That's bizarre. It's definitely not a personal document but a library loan which I pulled off of my Kindle, which I feel okay ripping since I own a hardback copy:
