I'm using Calibre 6.29.0* to convert a structured HTML file to EPUB, checking the results via Kindle Previewer 3. In the editor, I move the TOC file to follow a couple of frontmatter pages.

As I've been developing the book, I always see both the inline and sidebar TOC in Previewer.

Closer to the finish line now, I've emailed the Epub file to the Kindle apps on my Android and my Macbook. Neither show the sidebar TOC. (Actually, sidebar TOC on the Kindle apps is just two entries: (1) Go to beginning (2) Cover. The actual full sidebar TOC is missing.)

How do I troubleshoot this?

*This is the latest version of Calibre I can run on my Macbook.
