My Forma doesn't have a case - when not in use (or when travelling), it lives in a lightly-padded bookbag. Previous ereaders did have cases, not always an OEM case. My Kobo Touch lived in a Kindle case which fitted it perfectly.
At the moment, I rarely travel, so I don't see the need for a case. Besides, as with others, I hold mine one-handed, I'm short sighted, and I get tendonitis in both wrists so I'm very sensitive to the weight of a device.
The only ereader I ever broke was my Elonex when I fell asleep on the sofa, and put my elbow on it when I woke and tried to get up. I never managed to find a replacement screen.
