Moving from one Kobo to another is simple. Just copy over all KOReader and KFMON/NickelMenu files (if applicable) as well as all books and sidecar folders you were using with KOReader. Including the icons in the root folder of the device, .adds/koreader/* .adds/kfmon/* (if applicable) .adds/nm/* (if applicable). Make sure you retain the exact directory structure on the new device, and everything should work exactly as it did on your old device.
As far as the migration of the Calibre settings to the new device goes, I'm afraid I'm not able to help you with that. I suspect there is some config file for the old device in your computer's Calibre folder that you could adapt to the new device ID.
