That sounds as if something on your computer is interfering with calibre's temp files since it appears that titlepage.xhtml is being deleted. Antivirus/antimalware software is likely the most common suspect.

You could try using msconfig to disable all non-Microsoft services and then restarting to see if the issue persists. If the problem goes away, try adding back the non-Microsoft services in batches until the program re-occurs at which point you know the issue is with the last batch re-enabled. In the image below, you would click on Hide all Microsoft services and then Disable all.
