I think we will never know what happened. The symptoms match known causes, but you say none of those causes are possible.
What can be said with a great deal of certainty is that at some point some authors and/or titles were renamed, perhaps only a change in case. That caused calibre to attempt to rename the author/title folders, which apparently failed leaving dregs behind. These failures are always caused by some other software poking its nose into the middle of the process. The question is "what software"?
