You need to configure the driver.

Go to Preferences > Plugins. Search for KoboTouchExtended. Next click on Customize plugin. Click the Collections, covers & uploads tab. Then check Collections and Collections columns:. Now put in the columns you use for collections.. Also check Create collections and Delete empty collections.

That's all there is to do.
