Did you make an NCX type TOC.
(I don't think this is required)
Also, did you do a second connect.
I do know that if you change the structure of the book and send it again, you need to use Kobo Utilities Plugin to update the Kobo (it puts the TOC it uses into the DB) or you can get a ABEND if it tries for an non-existent section
