If you send the book from ADE using Send to computer/device, it goes into the Digital Editions directory. If you drag and drop, well, I can't do that from within ADE. If you select a book in the library view, a right click will give you the copy to computer/device option. But for me with ADE 4.5.11, I can't drag and drop to my mounted Kobo from within ADE. Now if I browse to the C:\Users\<username>\Documents\My Digital Editions directory, I can drap and drop from there but when I drop it on the Kobo reader, it drops it into the root of the exposed partition. I have to open the drive to see the internal directories to choose where to drop a file.

Given that KOReader is able to find it, that suggests that you are dropping it into a folder that Nickel's ExcludeFolder option does not allow Nickel to access or Nickel's import process does not run. Did you set up a directory for KOReader that is excluded for Nickel?

When you safely eject, do you see the Import process running so Nickel is importing the ebook to it's database?
