Not sure if this might be related to your problem but I once purchased an ebook that used only landscape orientation. Turned out in the opf document's metadata section, the creator has used:
Code:
<meta property="rendition:orientation">landscape</meta>
which I had thought was only for fixed layout but seemed to affect the reflowable epub I had.
