I can't remember if it was a Calibre setting, but I just have all of my books sent to a single folder on my Nook (no author directories) and then I set that folder as my Home folder in KOReader. Within KOReader, I set that folder to display in mosaic view to display a grid of the book covers.
Just a personal preference, but I have KOReader set to start with the last document that way it opens straight to whatever book I'm currently reading.
