Ok, I figured this out as far as I can tell.
- KCC converted files into .kepub.epub. However, on Windows, the filename just looked like .kepub (with the .epub hidden). I mistakenly thought it required additional conversion.
- Importing the .kepub.epub into Calibre, the manga shows up as a .epub, so I had to turn off autoconversion in KTE (by enabling .epub and .kepub files to be sent to the kobo).
- When sending the original KCC converted .kepub.epub, without another conversion to 'kepub' in calibre, the images do show in full screen mode without the header or footer.
- Specifically, the header is enabled for 'books', and manga shows full screen as desired.

In short: For full screen Manga, convert CBZ to kepub.epub in KCC, then either send directly to the device, or use Calibre. If using Calibre, be sure not to re-covert the files, just send as 'epubs'. This way, I could update the series, author, etc. in Calibre and still get full-screen pages.
