If you have the Kobo Utilities plugin installed, you can click on it to bring up the customization dialog. I've attached images of how I have that configured with the 4 custom columns.
If you are copying the databases back to the Kobo, you will need to copy the books back first and then copy the 2 databases since if the books are not found, they will be removed from the database.
When I do a factory reset, I copy the books back to the Kobo and then use Kobo Utilities to update the metadata and restore the bookmarks. I don't do anything with annotations at this time since most of annotations are errors in ebooks that I am correcting. After this is done, I will copy the databases back into place and do a reboot from NickelMenu.
And yes, the manual factory reset does the same as the factory reset from the Developer Options menu and wipes your Kobo back to how it came from the factory. The standard factory reset wipes the books but downgrade the firmware.
