Kovid (and probably user guide) explicitly states conversion to fixed layout ePub is not supported.
KCC does not create a table of contents for the ePub (which needs a human to decide which pages should be in it and what the labels are), so if you want one, you need to add one.
If you are starting with AZW3, Kindle Unpack does a good job converting to fixed layout ePub, and if will have whatever ToC the AZW3 has, which very often is not very useful. So you may want to add more entries to the ToC.
And neither will convert to PDF.
I like to have 4 formats available for comics (CBZ/ePub/PDF/KFX), so I mostly use Kindle Create. You can add table of contents, Panel View (if you want to side load KFX) and then convert the KPF to KFX comic format using KFX Output plugin. Then use the plugin export to generate CBZ, FXL ePub or PDF. The ePub, PDF, and KFX will have the ToC defined in Kindle Create. There are a couple of cosmetic issues I like to fix in the ePub and PDF output (takes less than a minute or two), but I have not gotten around to reporting the issues so that they can be addressed.
