Annotations with the pen on pdf files can be backed up just fine for side loaded pdfs.

Annotations with the pen on epubs end up as paired jpg and svg images in .kobo/markups , the jpg is the image of the page, the svg is your scribble. It doesn't matter if the epub was sideloaded or not, or if it is epub or kepub. I don't know if these are backed up to the cloud for cloud books. I'm sure there is a database entry that links the filename to the book or something, but I didn't look for it. The filename looks like a uuid.

Non-pen annotations (bookmarks, highlights, notes) can be exported to a text file which you can copy. I think calibre can also back those up separately with the annotations plugin, but it can't restore them. Integration of this could be better probably.
