You can delete the first four lines in that file and remove the # from the start of the #menu_item line. This will leave the file looking like this:

Thankyou! This worked, I now have the option to launch Koreader after selecting NickelMenu. I have not modified this file previously, I don't quite understand why this solved the issue..
Although Koreader now launches, icons are missing

I'm very close to just using the native Kobo interface, this seems like too much work for someone who is not familiar with programming and to be honest I don't really know what I am doing; the learning curve seems a little too steep.
It was an enjoyable few months when Koreader was working properly but I'm not sure its worth the effort if every Kobo firmware update results in issues such as the one I am experiencing, and having to waste people's time on this forum, which I am very appreciative of.
I guess I could just start from scratch and disable auto updates, something I should have done in the first place but eh.
