That should be the answer.
If you uncheck that box, the version on Kobo gets deleted and the new version gets sent. If title or other identifying information is changed, the new version will get sent but the old will not be deleted and you'll have both old and new on the device.
If that box is checked and certain things are changed, you'll get an error when trying to read the book on your Kobo.
