Since the original .azw file added to calibre which should automatically add the correct file extensions (.mobi, .azw3, etc.) will not open properly in calibre. See the OP's comment about "I can’t actually read the book in Calibre. Only the table of contents appears, and none of the actual content is accessible.". This together with the comments about how the .pdf looks suggests that the original file may not a reflowable format. KindleUnpack will extract the pdf file from an .azw4 Kindle ebook. If worst comes to worst, it will unpack the Kindle file so you can take a look at what files are present to get a handle on what the original format was.
