should it be 3 AM, 3 <smallcaps>AM</smallcaps>, 3 A.M., 3 <smallcaps>A.M.</smallcaps>, 3AM, 3<smallcaps>AM</smallcaps>, 3 A.M., 3<smallcaps>A.M.</smallcaps>, 3a.m., 3am, 3 a.m., or 3 am?
This kept me awake once. The last two are common and I added my decision to the flyleaf of The Penguin Dictionary of Abbreviations, which wasn't much help.
