Calibre will send covers to Scribe with MTP (for non print replica KFX), I think issues with this have all been fixed as of 7.14. And Kindle for Android will generate cover thumbnails from KFX (again non print replica), without any help from calibre (which AFAICT does not send any). So no need to call out MTP device as an exception.
As for the disappearing thumbnails, I've never been able to reproduce it. I think it was fixed with the date & time enhancement awhile back, while presumably older Kindles are still subject to it (I have one, but again cannot reproduce issue). I have not seen new reports, but none of that adds up to proof that it doesn't happen to some people somewhere.
So it remains true that
if they disappear, calibre will restore them when reconnecting.
