The press and hold has never worked, though the user guide says it should.
Calibre will look at the last part of the filename (.epub) and import the file as an epub. So when you imported the book, the filename/extension would become "The Railway Children - E. Nesbit.epub" in your calibre library.

To work around this, take your downloaded file, remove the .epub from the filename so it becomes 'e-nesbit_the-railway-children.kepub' and then import it. You will also need to install the KePub Metadata Reader to read the metadata from the file before importing it. When you send the .kepub to your Kobo, calibre will automagically add the .epub to the end of the extension.

For the most part, a kepub is simply an ePub with a mass of spans added used by the webkit based renderer on your Kobo for faster location.
