Indeed, I renamed the alt cover in my example... but there is no reason the kepubify process should be replacing jpg files. This wasn't a conversion. Somewhere in the repacking phase, files with the same name and filetype (but different extension) are getting mixed up. (The cover flag was properly set in the original e-pub, for both the title page and the jpg, but with this situation of image getting overwritten, the kobo did not like it. I did not examine the structure of the kepub further to see if the cover flag got misplaced as well.)
