I've copied your settings to an otherwise empty library and I don't see this happening.

I see the thread cover inside the epub and the GC cover in Calibre. Even after deliberately using Polish book to inject the GC cover and then updating, the epub has the thread cover.

You already covered most of the failure cases I can think of, so I'm rather at a loss. Some ideas are:

If you are doing one of the 'Update Calibre Metadata' update options that will not change the cover inside epub.

Trying to keep the ebook cover and the Calibre cover different probably isn't going to be easy. Calibre's Convert book, Send to device, and Save to disk all automatically inject Calibre's metadata and cover into the ebook as it is converted/sent/saved. And probably a few other places I've forgotten.

Oh! That's a good possibility: Inside Edit book, open Preferences, look for 'Integration with Calibre' (bottom left), and in that, see if 'Update metadata embedded in the book when opening' is checked. Because that will also inject the cover whenever you edit a book.
