The only circumstance I can think of this would happen is if you're changing the OEBPS/log_page.xhtml file. Everything else should be replaced. Or maybe a full disk, but I'd expect to see errors from that.
So if I'm going to help, I'll need the exact details to try and isolate what's happening: - What story URL are you using?
- What output format?
- Are you sure you're editing the same format file?
- Do you have FFF's 'Automatically Convert updates' feature on? Converting to what format? (It uses your Calibre Preferred output format setting.)
- What text in what file inside the ebook did you remove?
- Where did you change the author's name? It can be in an ebook more than one place.
- What are you using to check for the changes? Calibre's Viewer or something else?
- Are you sure you're looking at the same format FFF is writing?
- Are you sure you reopened the output format after overwriting?

1. The story itself hasn't mattered--I tried multiple stories from AO3 and FF.net to the same effect.

5. I changed the author's name in the Edit Metadata window
6. I used Calibre's Edit Book and Viewer to look for changes
7. There is only one format of the book - Epub

I've also restarted the program, the computer, and reinstalled the most recent test version.!
This is not a big deal though--I don't use this function often, and just wanted to let you know in case it was a widespread problem
