I suggest using Calibre (has its own Forum here) to create ePub and LRF. Actually it can also create a MOBI version, that is better than your existing one (e.g. no in-line TOC, more ePub-like styles if you want them), and will soon be able to create an eReader version.
I created an ePub and LRF from your HTML, and all I did was change class=subtitle to class=chapter (to allow Calibre defaults to apply, you can get Calibre to detect subtitle if you want), made the images local, and added the cover image from the MOBI. I don't know why the ePub text isn't wrapped around the image, as in the HTML version. Calibre usually preserves this, so it might be a Calibre bug which will be quickly fixed if you submit a bug report. The LRF (last screenshot) title font also looks a little weird. Even with these issues, which are likely fixable with minor edits to the HTML, I think your best bet is to get a good HTML version then convert it to the other formats with Calibre.
