The error you are seeing is caused by a bug in the KFX Input plugin that causes it to fail when converting Print Replica books using calibre 5.x versions. That will be fixed in the next release of the plugin.

You will need to update calibre in order to process that book if you cannot wait.

"From KFX" is built into the KFX Input plugin. Instructions for adding it to the main calibre toolbar can be found in the first post of this thread.
