Double chapters in Table of Contents

I'm converting a file to LRF.

I'm using <H2> </H2> around my chapter headers.

So this is exactly what's in the html code. <H2>Chapter One</H2>

When I convert to LRF, using the detect chapter headings and add to table of contents, the table of contents shows

Chapter One
Chapter One
Chapter Two
Chapter Two
and so on...

I converted another file and the Chapter headings worked...however the words within the H2 brackets aren't Chapter.

So it looks like

Monday
Tuesday
Wed

Because that's how the author labeled his chapter headings...

Basically, I have no clue why the word Chapter would trigger the program to make a double post when the exact same settings on another story don't?

I'd appreciate any and all help on this?
