You can't accidentally say yes: if you're offline and accidentally hit the "yes" button there will be yet another popup asking you permission to go out of airplane mode.

As per your second question, maybe reinstalling your actual firmware (and reapplying the hotfix) will work.

And I believe you can also rename the OTA bin to avoid any possible firmware update. This has also the advantage that, if that flamboyant update to read better japanese isn't actually a fw update, it might still work (not sure though, we don't know how it is going to apply it).
Btw, I would place my bet to "not a fw update", for the sole reason that fw updates on kindle do not get announced: they gets downloaded silently when you go online and applied at first reboot (or even sooner, in any case they definitely don't ask your permission).
