I would assume bad cable/port/controller + filesystem corruption and/or nickel choking on a broken file because of the broken transfer.
Nickel does absolutely *nothing* during an USBMS session, so it can only be a "physical" issue (or your OS sending a wonky eject or root hub power reset).
