This has nothing to do with Kobo Utilities.

In calibre, go to Preferences > Plugins. Search for KoboTouch. If you are using the KoboTouch plugin, select it and click Customize plugin. Go to the Collections, covers & uploads tab. Check Do not treat replacements as new books. If you are using the KoboTouchExtended plugin, do the same thing.

It will allow you to send the eBook to your Kobo without it being treated as a new eBook. The only time you need to use Kobo Utilities is if you've changed the ToC and need the ToC updated on your Kobo after you've side loaded the eBook.
