Or if you have multiple workspaces (an option even on XP). Calibre seems to open on whatever is the current workspace, but Chromium opens on whatever workspace it was last opened on, which gives the illusion of it not opening.
All sorts of weird stuff can happen, like if a power glitch as you move or close a window.
