This is a limitation/bug of the EPUBCheck Java tool which sometimes reports an error message without a file name.
Usually, it'll display other error messages for the same file before this error message. If you fix them, the error message without a file name will also go away.
Using the following options in Calibre Editor
before running the EpubCheck plugin might also help:
Tools > Fix HTML - all files
Tools > Arrange into folders
