The .ncx file is an ePub's 2 navigation document. It is not visible to the user but is used by the renderer when you are looking at the ToC. The other type of ToC is a text document which is pretty to look at but is not used by the renderer.

I've found most .azw3 books give me a cleaner epub2/3 file using KindleUnpack than calibre's conversion. OTOH, calibre's conversion will work with some azw3s that are poorly constructed. Best to have both tools handy.
