Are you discussing the ToC as seen from the .ncx file or a HTML ToC from a text file inside the epub. Have you tried using the KindleUnpack plugin to create the epub file rather than using calibre's conversion?

And for the love of bog, please wrap such long logs in spoiler and code tags.
