header line for kepubs- frustration

my reader settings are show pages left in chapter in header, pages left in book in footer
Kobo gets the numbers right, usually, but it is prone to latching onto one title tag ( like title page) and putting that into every chapter header, even when the chapter files each have Chapter name & number in their own title tags

if i directly select e.g. chapter 1 from the toc. the header will correctly say chapter 1, but go forward or back a chapter using the navigation arrows , and it reverts to title page in the header ( which it got from the title page foof the actual title page

in other books I have seen it render every chapter with a title of " part 1 " , or "cover", or "glossary"

I simply cant suss out the logic that the kobo software is using.
even when I carefully check all the known conditions it favours - such as 1 file per chapter, 1 header per file.. and a correct title tag set in each file, it still seems to go wrong

can anyone explain why please.

my kepubs are all generated from epub when sent via calibre using the correct driver plugin.
i can inspect aa copy of hte sent kepub and it will look to be correctly coded
e.g. 15th chapter file will start with
<head>
<title>Chapter 15</title>

but will still show in header on device , within said chapter , as title page 1 of xx

even more puzzling - with some books, it all just works, but I can't spot any difference in code between those books and the ones where it does not work so well

with the epub renderer the problem goes way , obviously, because there is no such header, but Id really like to understand the kepub issue - because every time it bugs me I want to go edit the epub and resend it to fix the issue.

but have inspected toc.ncx and content.opf files before sending - all looks correct in those.

so I am stuck

& cant find any relevant kobo documentation

is it a known kobo bug whereby it takes the content of the first title tag encountered in the book - or sometimes the last one in the book, and then always uses it instead of any actual title tag in the chapter

my kobo software version 4.35.204000 but AFAIK its always been this way in all firmware versions
