Also they often use source designed for PDF instead of source that's for epub and later editing a copy for PDF (paper print).
A word processor file can have no line-height in any style and use that to build ebook and proof. Then later edit all the pages (size, margins, registration, headers,footers) and styles to have line-height, margins etc for the paper format.
They do it the wrong way round!
